Physical exam forms for the 2024-25 school year are found at the links provided below. The exams must be completed after April 1 to be considered valid for the upcoming school year. Please note that the Sioux Falls School District has their own forms and students in that district SHOULD NOT use the below forms.
The South Dakota High School Activities Association will serve member schools by providing leadership in the development, supervision, and conduct of interscholastic activities which enrich the educational experiences of high school students.
