REGISTRATION IS FOR NEW USER ACCOUNTS ONLY!!!

  1. If you do not remember your username, please use the 'Forgot Username?' link to retrieve your username.  DO NOT create another user account.
     
  2. If you do not remember your password, please click the 'Forgot Password' link in the upper right corner of the screen to receive your login information via email (must enter your username to retrieve login information).

*NOTE: Once your account information has been submitted, your account will require authorization by an SDHSAA staff member. This authorization may take up to 24 hours to occur. You will receive email notification when your account has been updated/authorized. Once your account has been authorized, you will be immediately granted access to the portal environment. All fields marked with a red arrow are required. - (Note: - Registration may take several seconds. Once you click the Register button please wait until the system responds.)
